If you are interested in getting started, schedule the initial 45-minute video consultation. Once your requested appointment time is approved you will receive the intake form, a consent to treatment form, and maybe a few other forms I come up with.  Please be sure to fill these out and submit them at least 3 days prior to your scheduled appointment.

Also note: you will be required to keep a credit card on file within the ChARM system. This is a highly secure and HIPAA-compliant medical software system.

Cancellation policy

Please give me at least 24 hours notice prior to cancellation of any scheduled consultation. Cancellations less that 24 hours in advance will be charged $75.

• Consultations are done via Zoom (encrypted) or phone for those unable to Zoom.

• Available hours may change from week to week. Check back regularly to see my latest openings.

• Requested appointments will be confirmed typically within 24 hours.
